Background: This San Francisco woman came to Dr. Mabrie to seek resolution for areas of her nose that she was unhappy with from previous surgeries. Her nose was broken when she was 16, for which she had a Rhinoplasty surgery. She also had a Septoplasty surgery performed in 2000, for which they did a right collapse with deep inspiration. The results unfortunately left her nose appearing twisted and upturned.

 

Dr. Mabrie’s Evaluation: “It was immediately apparent, unfortunately, that she did have several post-surgical defects. On profile view, her nose is over-rotated and upturned, and has also been severely shortened. On frontal view, she has what is called an “inverted v deformity,” where the edges of her nasal bone can be seen pressing against the skin. The nasal tip and collumella have been severely twisted, giving a very unnatural appearance.

When addressing severe trauma from previous Rhinoplasty surgery, the goals of the revision surgery are very different than of first-time Rhinoplasty surgery. The scar tissue must be removed, and skin grafts are used to reconstruct the nose to restore a normal appearance.”

 

Procedure: Revision Rhinoplasty with increased tip projection, and lowering of nasal hump.

 

2 Month Follow-Up: On profile view the center is improved, smoother and more natural. On frontal view the tip and tip rotation is also improved. There is still some unnatural shadowing on the left; the patient and Dr. Mabrie are considering a second revision.
