‘Non Surgical’ Is Not the Same as ‘Temporary’ 

Dermal fillers are not a short-lived alternative to “permanent” surgery. In fact, they can be as enduring as any surgical enhancement. Here’s what many people don’t realize:

  • HA filler doesn’t dissolve on its own (though it can be reversed with hyaluronidase)
  • Over time, filler can integrate with your existing tissue
  • Filler has limited migration, staying relatively stable even as the rest of the face changes
  • To prevent overfilling, injectors need to account for residual filler and adapt their approach

While dermal fillers don’t require incisions or anesthesia to achieve their results, they still deserve the same care, expertise, and informed decision-making as a surgical procedure.

What We’ve Learned From the Research

A 2024 MRI study by Master et al. tracked 33 patients over 2.5 years and confirmed that residual HA filler remained in all 33 of them. These patients had not received filler injections for at least 2 years prior to the study, with most going 5 to 15 years since their last treatment.

In all of that time—and the 2 years under study—not a single patient showed complete filler dissipation in their imaging. What’s more, the volume of filler remaining was still significant in nearly 75% of patients.

Why Does Filler Last So Long?

Hyaluronic acid filler is chemically crosslinked for durability and often placed in tissue planes that are difficult for naturally occurring hyaluronidase (the HA-dissolving enzyme) to access. In addition, filler can integrate with the surrounding tissue, making it harder to break down naturally or uniformly over time.

These factors lead to a much slower, more challenging, and more unpredictable degradation process than was originally believed.

So, Is Long-Lasting Filler “Bad”?

In our view, long-lasting filler is more than just amazing—it’s worth celebrating! When done correctly, it’s the perfect filling choice, delivering long-term value and long-term results. Plus, it’s dissolvable! This last point is especially important, as bad results can persist as long as good ones.

The most common issue we see is injectors stacking new filler on top of old. In this scenario, patients receive:

  • The same volume of filler
  • At the same intervals
  • In the same place

Unsurprisingly, this approach leads to an overfilled, pillowy look over time—not because the patient had multiple filler treatments but because the injector hadn’t accounted for what was already there.

Dissolving Filler: When We Recommend a Reset

We use Hylenex® to dissolve hyaluronic acid filler when there is too much volume, incorrect depth or placement, or when patients have outgrown their current look. This concentrated injection of hyaluronidase breaks down filler much faster and more precisely than your own body can, effectively “reversing” the treatment.

Depending on how old the filler is or how much is present, this corrective procedure may take more than one session. We dedicate the same care to dissolving filler as we do to applying it: We evaluate, document, and plan, and we stage treatment as appropriate for your needs.

Only when you are cleared for retreatment do we proceed with adding new filler.

We recommend patients return for an evaluation every 9 to 18 months, so we can reassess their results and refine, if needed. These touch-ups are usually 10% to 20% of the original treatment, just enough for a natural-looking refresh.

Are long-lasting fillers a problem?

Only if they’re placed incorrectly. Since fillers can last for years, precision placement and conservative maintenance are essential.


Can you dissolve filler?

You can dissolve hyaluronic acid filler with an enzyme called hyaluronidase. While the process starts working within minutes, you may need multiple sessions to fully clear the old filler.


Why doesn’t filler dissolve on its own?

The naturally occurring hyaluronidase in your body isn’t robust or concentrated enough to break down HA filler effectively on its own. Hyaluronidase injections are more targeted and offer a faster, more predictable breakdown of filler.
