How Long Does Filler Last?

Hyaluronic acid (HA) filler can last for a decade or more, far longer than the typically quoted range of 6 to 18 months. This revelation is groundbreaking for our profession, but it reinforces what we have known for decades: The best filler results come from careful planning, conservative refinements, and a long-term perspective.

As pioneers in non surgical rejuvenation, Mabrie Facial Institute in San Francisco has spent more than 20 years challenging the conventional wisdom and pushing the boundaries of what injectables can achieve. What we now see in the emerging research on filler longevity is what we have observed across thousands of patients at our own practice—filler is a long-term commitment, not a short-term fix.

‘Non Surgical’ Is Not the Same as ‘Temporary’ 

Dermal fillers are not a short-lived alternative to “permanent” surgery. In fact, they can be as enduring as any surgical enhancement. Here’s what many people don’t realize:

• HA filler doesn’t dissolve on its own (though it can be reversed with hyaluronidase)
• Over time, filler can integrate with your existing tissue
• Filler has limited migration, staying relatively stable even as the rest of the face changes
• To prevent overfilling, injectors need to account for residual filler and adapt their approach

While dermal fillers don’t require incisions or anesthesia to achieve their results, they still deserve the same care, expertise, and informed decision-making as a surgical procedure.

What We’ve Learned From the Research

A 2024 MRI study by Master et al. tracked 33 patients over 2.5 years and confirmed that residual HA filler remained in all 33 of them. These patients had not received filler injections for at least 2 years prior to the study, with most going 5 to 15 years since their last treatment.

In all of that time—and the 2 years under study—not a single patient showed complete filler dissipation in their imaging. What’s more, the volume of filler remaining was still significant in nearly 75% of patients.

Why Does Filler Last So Long?

Hyaluronic acid filler is chemically crosslinked for durability and often placed in tissue planes that are difficult for naturally occurring hyaluronidase (the HA-dissolving enzyme) to access. In addition, filler can integrate with the surrounding tissue, making it harder to break down naturally or uniformly over time.

These factors lead to a much slower, more challenging, and more unpredictable degradation process than was originally believed.

So, Is Long-Lasting Filler “Bad”?

In our view, long-lasting filler is more than just amazing—it’s worth celebrating! When done correctly, it’s the perfect filling choice, delivering long-term value and long-term results. Plus, it’s dissolvable! This last point is especially important, as bad results can persist as long as good ones.

The most common issue we see is injectors stacking new filler on top of old. In this scenario, patients receive:

• The same volume of filler
• At the same intervals
• In the same place

Unsurprisingly, this approach leads to an overfilled, pillowy look over time—not because the patient had multiple filler treatments but because the injector hadn’t accounted for what was already there.

Dissolving Filler: When We Recommend a Reset

We use Hylenex® to dissolve hyaluronic acid filler when there is too much volume, incorrect depth or placement, or when patients have outgrown their current look. This concentrated injection of hyaluronidase breaks down filler much faster and more precisely than your own body can, effectively “reversing” the treatment.

Depending on how old the filler is or how much is present, this corrective procedure may take more than one session. We dedicate the same care to dissolving filler as we do to applying it: We evaluate, document, and plan, and we stage treatment as appropriate for your needs.

Only when you are cleared for retreatment do we proceed with adding new filler.

Nose Filler (Non Surgical Rhinoplasty)

A liquid rhinoplasty lets us fine-tune the nose for better proportion and facial harmony. In many cases, patients are looking to avoid surgery, while others want to complement—or correct—their previous surgical results. Surrounding areas may also be addressed, either initially or down the line, to keep the face in balance.

Lip Filler

The lips are one of the easiest areas to overfill, especially for inexperienced injectors who perform the same treatments on repeat. To avoid duck lips or “trout pout,” our Injectors take a conservative approach to lip filler, emphasizing balance and definition over volume alone.

For patients coming to us from another practice, we may need to dissolve excess or misplaced filler before refining results.

Chin & Jawline Support

Similar to our approach to the midface, we consider how your features work together to create support. We use dermal fillers to add projection to the chin and sharpen a jawline, creating a clearer transition between the face and neck.

Chin Augmentation With Filler

Chin shaping with filler refines the profile and subtly harmonizes the face. This treatment is rarely limited just to the chin. Based on your Illumination 360° evaluation and changes over time, we may pair chin augmentation with jawline contouring or midface support.

Smile & Marionette Line Support

This region is where support and expression intersect. We evaluate refinement in this area through an architectural lens, focusing on structure rather than spot treating.

Keep Your Glow Going With Dermal Filler Maintenance

We recommend patients return for an evaluation every 9 to 18 months, so we can reassess their results and refine, if needed. These touch-ups are usually 10% to 20% of the original treatment, just enough for a natural-looking refresh.

As you saw above, a good maintenance plan needs to be flexible. Patients often find that as the years go by, they need support in additional areas, not just the original treatment site. In these cases, maintenance is as much about restoration as it is rebalancing over time.

Our young patient’s initial focus was the chin, lips, and nose. As she gets older, her treatment plan has called for refinement in the midface and cheeks, the under eyes, and the jawline. The evolution is subtle, yet the results remain seamless.
