If you don’t have movie star cheekbones, you’re not alone—this is an extremely in-demand feature for both women and men. But high, prominent cheekbones don’t come naturally to many of us, either due to genetics, age, or fullness in the lower cheeks. If you’re not interested in surgical implants, you can still have the cheeks of your dreams with fillers and BOTOX®. At Mabrie Facial Institute, our “San Francisco aesthetic” means we sculpt your cheek contours to look natural and balanced, never overdone or obvious.
Creating or restoring the cheek contours you want requires adding (or sometimes subtracting) volume beneath the skin. The highly trained Injectors at MFI specialize in using cheek filler and neuromodulators to lift and shape flat cheeks and bring prominence and angularity to undefined cheekbones.
Today’s fillers are truly remarkable, and products like JUVÉDERM® VOLUMA® and Restylane® Lyft are made specifically to enhance the cheeks. They feature a thicker formula of hyaluronic acid, which makes them more durable and appropriate for adding structure to the cheekbones.
If your cheeks have flattened, drooped, or become hollow over time, fillers can be used to re-create your more youthful cheek fullness. We often do this as part of our Midface Rejuvenation procedure, which restores the areas adjacent to the cheekbones, including nasolabial folds and under eye bags and circles.
If fullness in your lower face is disguising your cheekbones, it may be due to enlarged chewing muscles. We treat this with our BOTOX & Filler for the Round Face package to slim the lower face, augment the cheekbones, and add definition to the jaw.

Our BOTOX for the Round Face package includes jaw slimming, cheek filler, and chin & jawline filler, with up to 4 syringes of filler and 20 units of BOTOX. Once you’re happy with your cheek contours, you move into the maintenance phase of treatment. From this point on we only need to see you once every 9 to 12 months to touch up the midface and/or cheeks, typically only using 1 to 2 syringes.

If your cheekbones have always been low, flat, or undefined, then you can probably blame genetics. The prominence of your cheekbones is a factor of the ratio of your zygomatic bone and the surrounding soft tissue (cheek skin and fat).
Flat, hollow, or sagging cheeks can be the result of age-related volume loss caused by fat breakdown and bone resorption. If you have flattened, sagging cheeks, volume loss may be the culprit. You may also have bottom-heavy cheeks caused by sagging skin and fat collecting below your cheekbones.
If the most prominent part of your cheek is the area beneath the cheekbone, your cheeks will appear bottom heavy. This can create the look of a “baby face,” or “chipmunk cheeks.” Bottom-heavy faces are caused by sagging skin, underdeveloped cheekbones, and/or overdeveloped masseter muscles or parotid glands. We treat bottom heavy cheeks with filler to the cheekbones, chin, and jawline, and BOTOX to the masseter muscle—all included in our BOTOX & Filler for Round Face package.
We’ll sculpt your ideal cheekbones at our chic, upbeat San Francisco office, just off Union Square. You’ll have a candid consultation with one of our exceptional Injectors to discuss your concerns and your goals. They will carefully examine your face from every angle, under different lighting, to determine the best approach for achieving the look you want.
Please set aside up to 2 hours for your first visit, which includes consultation, prep, treatment, pre- and post-treatment photography, aftercare education, and scheduling your next appointment. The injections themselves only take a few minutes and cause minimal discomfort.
Our skilled Injectors place cheek fillers along the zygoma (cheekbone) to shape and form the outer cheekbone prominence along the upper midface. If filler is placed too low on the face, it can create a bottom-heavy appearance. We carefully select the products we use, choosing different consistencies for different contouring effects. If you’ve opted for our BOTOX & Filler for Round Face package, we’ll also perform neuromodulator injections and chin/jawline filler at your first appointment.
Your cheek filler procedure may require 2 or more appointments. This staged approach allows us to create structure, support, and truly remarkable yet natural-looking, healthy results. While we can accomplish a lot during one appointment, we have learned that a “layered” approach works best.
At your second appointment, scheduled 1 to 3 months after your initial appointment, we carefully evaluate the new contours of your cheeks. Since all swelling has subsided, we can see clearly where we need to layer on more filler for optimal contours. This is also the time to add filler to more delicate areas like the under eye. The second appointment is our opportunity to build more filler onto the foundation laid at the first appointment. This is how we achieve the amazing definition and contours our patients love.

For the best experience, follow these tips in advance of your appointment:
Some common injection-related reactions may occur, such as swelling, bruising, pain, itching, discoloration, and tenderness at the injection site. These are normal side effects and are most apparent 24 to 48 hours after treatment. These typically subside within 2 to 5 days. Most patients return to their routine activities the same day.

Our preferred fillers are JUVÉDERM VOLUMA XC and RADIESSE®. RADIESSE lasts up to 1 year with touch-ups at 6-month intervals, and JUVÉDERM VOLUMA XC results can last up to 2 years. For patients who desire short-term results, we offer fillers such as Restylane Lyft, which last up to 6 months with touch-ups at 3-month intervals. Neurotoxin results typically last 3 or 4 months. Most of the fillers we use for cheek and chin enhancement are hyaluronic acid-based. This means they are reversible with an injectable enzyme called Hylenex, meaning we can reduce or erase your results if you change your mind. RADIESSE cannot be dissolved.
Generally, we use Restylane as our go-to tear trough filler and JUVÉDERM VOLUMA is our favorite of the cheek fillers.
