Filler for Wrinkles Under the Eyes
While under eye wrinkles themselves are challenging to address directly, they typically are NOT the main cause of an aged appearance but rather just a small contributing factor. At Mabrie Facial Institute, we find that, in most cases, it’s dark circles, sunkenness, and bags under the eyes that are causing the aged appearance. Addressing those signs with under eye filler will support the skin beneath the wrinkles and give a brighter, fresher appearance. It may also improve the appearance of the wrinkles in some instances.
This is an example where filler improved dark circles and reduced bags under the eyes. Although wrinkles were not necessarily improved, her appearance is refreshed and youthful.
This is a good example of a person whose wrinkles weren't erased but who still achieved a more youthful appearance by adding volume to the under eyes and reducing dark circles.

As fat and bone in your midface break down over time, the lack of support causes the area beneath the eyes to sink, droop, and wrinkle. The result is hollowing of the tear trough area and sometimes bags—pockets of displaced fat. These contour irregularities and the shadows they create are usually the main reasons for an aged under eye appearance, not the fine lines and wrinkles.
Thinning Skin
With age, the skin undergoes a decrease in the production of structural proteins, including collagen and elastin. This causes skin to become thinner and less elastic, contributing to crepey skin texture. This is especially true of the extremely thin skin under the eyes.

Consultation and Planning for Your Under Eye Wrinkle Treatment
Your First Visit
To begin your journey to smoother under eyes, you’ll come into our beautiful, friendly San Francisco office to meet with one of our experienced Injectors. We’ll discuss your concerns and goals, then take professional-level photos from all angles. Based on our conversation, the examination, and the photos, we’ll determine the best treatment for achieving the results you want.
Please plan to spend about 1 to 2 hours in the office for your first visit, which includes consultation, prep, treatment, pre- and post-treatment photography, aftercare education, and scheduling your next appointment.
Your Under Eye Wrinkle Filler Procedure
Our exceptionally trained Injectors carefully introduce small, safe quantities of filler for results that never look artificial or overdone. At MFI, we use a blunt-tipped microcannula, which minimizes bruising and lowers the risk of complications. We place the microcannula just under the skin and use a fanning technique for precise filler application. We almost always stage treatment across 2 appointments.
Your Treatment for Under Eye Wrinkles Takes Two Treatments
At MFI, under eye filler requires a minimum of two appointments. This staged approach allows us to first establish structure and support and then make refinements for the best results. We start conservatively, then build and layer until we get it just right.
For the best filler experience, follow these tips in advance of your appointment:
A few days before your appointment, restrict blood thinners, such as anti-inflammatories, fish oil, and alcohol. Limiting these will reduce your risk of bruising from under eye filler.
DO NOT discontinue prescribed, medically necessary medications.
Eat and drink a normal breakfast or lunch prior to your appointment; low blood sugar can make patients lightheaded during treatment.
Recovery & Aftercare
Immediately after filler under your eyes, you may experience swelling, bruising, tenderness, and redness. These are normal side effects and are most apparent 24 to 48 hours after treatment. They can last several weeks but typically subside within 2 to 5 days. Most patients return to their routine activities the same day.
Swelling
Initial swelling may cause the eye area to appear full or puffy, with maximum swelling the day after treatment
This may cause temporary asymmetry, lumps, or bumps
Will gradually resolve to reveal your final results over 4 to 6 weeks
Bruising
While bruising is possible, it is usually minimal

What is the best filler for treatment of under eye wrinkles?
Since the skin under the eyes is very thin with minimal fat beneath it, the best filler for the under eye or tear trough area is a soft, moldable, hyaluronic acid filler. We use Restylane-L® to add volume under the eyes.
Which is better for under eye wrinkles, BOTOX or filler?
BOTOX is the go-to treatment for crow’s feet—the branch-like wrinkles at the outer corners of the eyes. That’s because they are dynamic wrinkles, meaning they are caused by repeated facial expressions. For finely wrinkled skin directly beneath the eyes, we find that filler, especially to smooth underlying hollows or bags, is the best solution. Once the volume loss is corrected, often the appearance of the wrinkles is improved.
How long will my results last?
Our patients usually enjoy results in this area that last a year or more. We recommend maintenance treatments approximately every 9 to 12 months so that you can remain looking your best indefinitely. The amount of filler required at these appointments is usually significantly smaller than during the initial stages of treatment.
