Background: This young woman consulted with Dr. Mabrie about her eyes, which were slowly looking older. She was noticing more wrinkles above and below the eyes and met with Dr. Mabrie to discuss upper lid and lower lid blepharoplasty. Importantly, she wanted to preserve the general shape of her eyes and was concerned that surgery might change how her eyes looked.

Dr. Mabrie's Assessment: "This case was very interesting because it demonstrated how complex the eyes can be. This woman was still very young but had started to develop wrinkles around her eyes. The upper lids were pretty straight-forward, she had a small amount of excess skin. I knew exactly what she meant about preserving the overall shape. When excising skin, it's important to discuss the desired shape because The surgeon may change the shape depending on how much skin is taken. 

Most patients that consult with me for the lower lids are able to be treated with fillers alone to the tear trough, and sometimes to the cheeks. Occasionally, patients have some bulging fat which must be addressed with lower lid blepharoplasty. In this case, she didn't have bulging fat, but had a lot of loose skin. So we made a plan to perform a skin pinch procedure and follow up with fillers to build up her cheeks and smooth the transition from the eyes to the cheeks."

Procedure: Upper Lid Blepharoplasty Lower Lid Blepharoplasty (skin pinch technique) and Restylane to the tear troughs and cheeks.

Follow Up: Both Dr. Mabrie and the patient were very happy with the result. Her upper lids look better, without the wrinkles, but maintaining her natural shape. The lower lids are now smooth and her cheeks are great.
