Under-eye circles, hollows, and bags; nasolabial folds; and sagging cheeks all top the list of concerns that bring people to our San Francisco Bay Area practice for dermal fillers. Many of us have these problem areas that can make you look tired and add years to your face. It’s natural to want to “fix” them by injecting filler to the area (bad idea—we’ll get to that later). But these conditions are actually just symptoms of an underlying issue that’s not as visible. The real culprit is volume loss in the midface, which must be addressed as a zone to restore a smooth under eye area and reduce the prominence of nasolabial folds. Do You Need Midface Rejuvenation?

The idea of treating the midface as 1 zone often causes an “Aha!” moment for our patients.
What exactly is the midface? This zone or area consists of several facial areas starting at the tear trough, extending outwards to the cheek then continues downward encompassing the mid-cheek to the corner of the mouth. Treating this area as 1 zone is key for the most natural and refreshed results versus applying filler directly to the singular areas. Why?
Think of your midface like a soufflé or cake: When the middle sinks, the cake deflates down and inward, pulling on the edges causing them to flatten and buckle. The same occurs when the midface loses its volume. The bordering areas (under eyes, cheeks, and nasolabial folds) are impacted by the loss of support in the midface (the center). When this volume loss occurs, the secondary effects can be more noticeable than the underlying cause.

Problem 2: Nasolabial Folds


The nasolabial folds—the crease of skin from the nose to the corner of the mouth—are a frequent concern for the still-young-but-starting-to-age population. Everyone has these lines, but when the cheek skin is not supported by midface and cheek volume, it falls downward and forward causing the nasolabial fold to become deeper and more pronounced.
Like the eyes, the nasolabial folds are commonly mistreated with a syringe of filler placed directly into the crease or groove that can actually make the fold more noticeable. The nasolabial folds are best corrected by treating the entire midface area with filler. Restoring volume to the midface lifts the skin, greatly diminishing the nasolabial fold.
Problem 3: Sagging Cheeks (Premature Aging)


The aging face usually starts with mild under eye bags and nasolabial folds caused by volume loss in the midface. As the volume continues to degenerate, the effects are more severe, causing the cheeks to sag or droop.
Restoring volume to the midface creates vertical lift to smooth sagging cheeks of the aged face. This also diminishes the nasolabial folds and under eye hollows/bags.
What’s Wrong With Just Treating the Single Area?
The simple answer is, treating a single area on its own is ineffective and could make the area look worse. Most patients and many providers, attempt to solve under eye and nasolabial fold concerns by placing filler where you see the line/shadow/bag. This is the wrong approach because it doesn’t address the root cause of the problem. When filler is placed incorrectly, it can cause a drooping/puddle-effect that can make your initial concern more exaggerated creating heavier under eye bags and nasolabial folds—the exact opposite of what you want. This leads to a high level of patient dissatisfaction and creates a belief that “fillers didn’t work on me.” Properly placed filler, in the right hands, can be an effective solution for both women and men of varying skin types and ages.

Your Midface Rejuvenation Procedure
We take a holistic approach to fixing dark circles, bags, smile lines, and sagging cheeks, and our patients love the results. Using several types of fillers, we focus on restoring volume to the midface and the adjacent areas, and we stage this process over 2 (or more) appointments.
Initial Appointment
Treatment begins by restoring volume to the larger anatomical areas—the midface and cheeks—to address primary volume loss and/or bone resorption. This serves to create the “foundation” that we’ll build upon at your refinement appointment.
We wait 1 to 3 months between treatments to allow for swelling to subside before placement of the next layer of filler, only where it is needed.
Refinement Appointment
At this appointment, we evaluate the results of your initial treatment, then build upon that initial foundation. We can now directly place filler in the smaller areas—such as the tear trough or nasolabial folds—knowing that we have the support in place to keep it there. This is where you’ll really see your results take shape, however, you may still require a third appointment for optimal results.

Midface Rejuvenation FAQs
How much does midface rejuvenation cost?
The midface rejuvenation package price is $3,500-$4,500 (depending on Injector). This includes your first appointment and the products used, as well as your second appointment and one additional syringe of filler. If additional syringes are needed, those are priced at a reduced-rate.
0% financing for 6 to 12 months is available for qualified borrowers through CareCredit and Alphaeon medical financing.
How long do dermal filler results last?
Depending on which fillers are used and which areas are treated, your results can last from 6 to 18 months or longer. Dermal fillers are temporary products that require touch-ups to maintain results. We recommend maintenance appointments at 9- to 12-month intervals to keep your results looking fresh. What is recovery like after dermal filler injections?
Immediately after treatment, you may experience swelling, tenderness, redness, and bruising. These are normal side effects and are most apparent 24 to 48 hours after treatment. They can last up to 2 weeks, but typically subside within 2 to 5 days. Because we know you have questions about after care, we created an entire page dedicated to Understanding Your Recovery Process. Most patients return-to-work same-day.
Which fillers do you use for midface rejuvenation?
Generally, for under-eye rejuvenation, we use Restylane® for the tear trough and JUVÉDERM VOLUMA® for the cheeks and midface.
